Re: Required Physical Exams For All 7th Graders Notification

Aloha ULS Parents and Guardians,

The Hawaii State Departments of Education (DOE) and Health (DOH) are collaborating on ways to protect and ensure the health of our keiki. We know adolescence is a time of tremendous physical, emotional, and social change, and a physical examination can help to prevent health issues from developing.

State law requires that before the first day of school, all seventh-grade students must provide documentation of having received a physical examination and the following vaccinations:

  • Tdap (tetanus, diphtheria, pertussis vaccine)
  • HPV (human papillomavirus vaccine)
  • MCV (meningococcal conjugate vaccine)

Verifying The Physical Exam 

A Form-14 can be found at the following link (Student’s Health Record Rev. 2018), or on the school website, or from your healthcare provider.  This form will verify that your child has had the required physical exam for grade 7.

Please submit the completed Form-14 by July 23, 2021.  If you are unable to turn in the form by the due date, make sure to turn in complete form by the first day of school.
